Court Forms & Fees

The court system has many free forms for you to use. But, there may not be a form for what you need. Also, there may be a fee to file a form in court. This section give you links to find out about court forms and fees and what to do if you don’t have money to pay a fee. All court forms must be typed or printed neatly in English, in black ink, on 8 ½ x11 inch paper, double spaced, using one side of the paper only. Papers should be stapled together and the original forms should be filed in the court. If you need help finding a form you can use the court locator box to call a court or visit a Court Help Center.
