Disciplinary History Reports

A Disciplinary History Report is an official record of the Attorney Grievance Committee for the Third Judicial Department ("Committee"), concerning attorney professional misconduct matters, which have been reviewed and determined by the Committee and/or the Appellate Division, Third Judicial Department. A Disciplinary History Report provides an attorney's current registration status with the Office of Court Administration Attorney Services and determinations issued by the Committee and/or the Appellate Division, Third Judicial Department.

Only an attorney may request a Disciplinary History Report. A request from an individual other than the requesting attorney will not be processed. A Disciplinary History Report will be sent by email to the requesting attorney's email provided in the request.

If you are requesting a Disciplinary History Report in connection with your admission application to a State Bar, please submit to this Office at the address listed above a pre-addressed, stamped envelope. Upon receipt of a proper envelope, the Disciplinary History Report will be forwarded to the State Bar.

Please allow up to 30 days for processing a Disciplinary History Report.
