Print Intake Form

You can print out copies of the VLP Intake Form, which explains to the self-represented litigant the scope of your involvement. Make sure to discuss the form’s contents with the self-represented litigant when you meet. After it is signed, provide the litigant with a copy. Keep a copy of the completed form for your records, and provide a copy to the Resource Center Administrator at the front desk. If you do not bring the intake form with you, please remember to ask the Resource Center Administrator for a copy of the form, which is available in triplicate.
