File this form in District Court, City Court, Town Court or Village Court.

You can use this program if:

  • You do not have a lawyer, and
  • The premises is unregulated housing, and
  • The premises is not owned by a corporation or voluntary association, and
  • You are the owner of the premises, and
  • You own less than 3 rental units, and
  • The people you want to evict moved in without permission from you or anyone else.

 

Information Checklist

You will need the following information with you when you use this program:

  • The name and address of the tenant.
  • It is helpful to know the exact date or your best guess for the date that the squatter moved into the premises.
