SUFFOLK COUNTY - Surrogate's Court

Need an ADA Accommodation? 

To request one: 

  • use the new online ADA Request for Accommodation Pilot form for Suffolk County, 10th Judicial District.

  • for Surrogate's Court send an email to the District Executive at ADASUFFOLK@nycourts.gov. Please put "ADA Accommodation Request" in the subject line.

  • for Commissioner of Jurors send an email to the Juror personnel at suffolkjury@nycourts.gov. Please put "ADA Accommodation Request" and the "Juror Index Number" in the subject line.

ADA ENTRANCES: Main Entrance: Entrance is located at Center Drive South and Evans K. Griffing Drive . Entrance is accessed by a ramp and assistance can be provided by staff.
